COUNTRY CARDIO® DANCE EXPERIENCE

PARTICIPANT AGREEMENT, RELEASE OF LIABILITY, & PROPRIETARY CONTENT AGREEMENT & LICENSE

Please read this document carefully. You agree to all terms and conditions required for admission to this class.

LIABILITY WAIVER & PHYSICAL RISK ACKNOWLEDGMENT

1. I understand that Country Cardio® is a steady-state cardio and resistance training program. I acknowledge that participation involves physical exertion, which carries an inherent risk of injury. I voluntarily assume all risks associated with my participation. I expressly agree to release, indemnify, and hold harmless Heather Lea Gerdes, Country Cardio®, and the host studio/organization from any and all claims, demands, injuries, damages, or causes of action arising out of my participation in this class.

2. RESISTANCE EQUIPMENT (ROPES) VOLUNTARY USE

This class may involve the use of resistance tubes, proprietarily referred to as "Ropes." I acknowledge that using resistance equipment carries risks, including snap-backs, breakage, muscle strains, or equipment slipping. I agree to use the Ropes safely, at my own risk, and according to the instructor's explicit guidance.

3. PHOTO, VIDEO, & MEDIA RELEASE (IN PERPETUITY)

I understand that Heather Lea Gerdes and authorized personnel will film and take photographs during this class. By attending, I grant Heather Lea Gerdes, Country Cardio®, CountryCardio®Club, and CountryCardio®Boomers the absolute, unrestricted, and irrevocable right to use, publish, edit, and distribute my image, likeness, voice, and video recordings across these aforementioned media channels, social media accounts, and any other media format worldwide, in perpetuity, without compensation or limitation.

4.) INTELLECTUAL PROPERTY, TRADEMARK, & COPYRIGHT ENFORCEMENT

I acknowledge that Country Cardio® is registered trademark and CountryCardio®Club, and CountryCardio®Boomers are proprietary properties of Heather Lea Gerdes. All choreography, Signature Steps, training manuals, concepts, artistic designs, logos, apparel, and products are the exclusive intellectual property and copyrighted works of Heather Lea Gerdes and Heather Lea Gerdes Design.

Whether you are a fitness professional, instructor, or enthusiast, you are strictly prohibited from teaching, duplicating, or using any Country Cardio® methods, materials, or branding without official certification and a valid license from Country Cardio® and/or Heather Lea Gerdes. Heather Lea Gerdes is fully prepared to legally enforce trademark and copyright infringement to the furthest extent of the law.

5.) IN-CLASS FILMING POLICY & MUSIC LICENSING

I understand that licensed music may be utilized during this class for instructional purposes. To protect intellectual property, proprietary choreography, and music licensing guidelines, student filming or recording during the active class session is strictly prohibited. Opportunities for photos or videos may be provided after class or during a Q&A session.

6.) VOLUNTARY GRATUITIES (TIPS) & ADMISSION POLICIES

I acknowledge that this Dance Experience may be offered without a mandatory admission charge. Voluntary tips/gratuities may be accepted and appreciated. I understand that adherence to all rules in this document is a strict requirement for admission, and the instructor reserves the right to deny entry to anyone who does not agree to these terms.
